Our Database of Medicare and Medicaid Certified Nursing Homes shows the latest results of inspections by the federal government and contains demographic data about the residents, including each facility's percentage of residents who:

 

 
Level1: No actual harm with potential for minimum harm. The violation has had, or has the potential for having no more than a minor negative impact on the resident(s)
Level 2: No actual harm with potential for more than minimal harm that is not immediate jeopardy. The violation has had minimal negative impact on the resident(s) or it has the potential to affect the ability of the resident(s) to reach their highest level of well-being
Level 3: Actual harm that is not immediate jeopardy. A resident has, or residents have, been negatively impacted physically and/or emotionally or psychologically
Level 4: Immediate jeopardy to resident health or safety. Residents have suffered or are likely to suffer serious injury, impairment, harm
 
 

 

* Need for Help With Daily Activities Has Increased
* Have a Urinary Tract Infection
* Who Lose Control of Their Bowels or Bladder
* Have Moderate to Severe Pain
* Who Spend Most of Their Time in Bed or in a Chair
* Were Physically Restrained
* Ability to Move About in and Around Their Room Got   Worse
* Are Depressed or Anxious
* Who Have/Had a Catheter Inserted and Left in Their Bladder

 

And following stats for each facility, compared to state and National Averages:

 

* Number of Residents , 
* RN Hours per Resident per Day, 
* LPN/LVN Hours per Resident per Day
* CNA Hours per Resident per Day, 
* Total Number of Nursing Staff Hours per Resident per Day

 

The database also shows faults inspectors found in each facility and a "Level of Harm" rating for each infraction.  The rating key is on the left side of the page.
